Hong Kong residential care worker schemes: care worker vs health worker vs nurse

Direct answer: residential care workers are not filed under the general ESLS route — the SWD Importation of Care Workers Scheme manages quotas, and the Immigration Department expressly defines the scheme's care worker as excluding care assistants, health workers and nurses. Health workers must be registered under the residential-care homes regulations and nurses must hold a valid practising certificate — the three roles cannot share one importation route.

This page is a knowledge guide to help readers identify the applicable route; eligibility follows the official current requirements.

1. Direct answer: identify the role first, then the route

Hong Kong care roles follow separate importation routes by role level, and merging them is the field's most common misreading. Four role types, per the official definitions:

RoleQualification and registrationImportation route
Residential care workerQualifications and experience for the care worker post; pay not below the monthly median (SWD scheme figure: HK$15,500)SWD Importation of Care Workers Scheme quota — the ImmD definition expressly excludes care assistants, health workers and nurses
Health workerMust be registered under the Residential Care Homes / Residential Homes for Persons with Disabilities Regulations — completing an approved course is one route to qualifying (SWD registration notes)Not within the scheme's care worker definition; verify the applicable route by institution type and actual duties
Nurse (registered / enrolled)Registered or enrolled, and holds a valid practising certificate issued by the Nursing CouncilOutside the scheme; verify by registration and practising requirements
Hospital support roles (colloquially 'hugong')Formal job title and duties prevailDo not map to the residential scheme by nickname; verify by institution type

2. Who can use the residential care worker scheme

Under the SWD Importation of Care Workers Scheme, residential care homes may import care workers while local workers keep employment priority: eligible homes are licensed elderly homes under the Residential Care Homes Ordinance (Cap. 459), licensed residential homes for persons with disabilities under the Cap. 613 ordinance, and infirmaries holding exemptions. Quota applications are filed within designated application windows (about two weeks each) through the SWD application and quota management system; recruitment from the mainland must go through mainland labour enterprises approved by the Ministry of Commerce for Hong Kong labour cooperation. Review takes about two months, and pay must not be below the care worker monthly median (SWD page figure: HK$15,500).

The boundary matters equally: the scheme does not cover hospitals, clinics or home-care companies — nursing and support roles at those institutions must be verified by institution type, formal title, actual duties, registration requirements and the applicable importation arrangement.

3. How to write the role description

A care role description should state item by item: the formal/statutory job title, the residential home, the scope of personal-care duties, shift and night arrangements, normal hours, rest arrangements, pay and overtime calculation, accommodation, required experience and training, and the reporting line. Do not merge nursing, nurse work, health-worker duties and cleaning chores into one endlessly expanding post — the vaguer the duties, the harder the qualification checks and reviews.

4. What job seekers should verify

Before taking a care role, verify item by item: the employer and the home's identity (licensed? who applies?), the actual role and duties, the required qualification (registration or experience), the workplace, contract term, pay and who bears which costs, accommodation, and the recruiter's identity. Two bottom lines: 'care experience' does not by itself equal nurse or health-worker qualification; and any pre-signing fee should be itemised with the charging party and basis made clear.

5. Credentials and training in layers

'I already hold a mainland care certificate — what else is needed?' Sort credentials into four layers. Layer one: statutory registration (health-worker registration; nurse registration/enrolment and practising certificate) — assessed individually by the SWD and the Nursing Council under Hong Kong standards; mainland credentials are not automatically equivalent. Layer two: the importation scheme's qualification requirements (the scheme requires qualifications and experience for the care worker post). Layer three: the home's operational training requirements for staff. Layer four: the employer's additional role requirements. The layers are independent — 'get a care certificate and go to Hong Kong' is not how it works; each layer follows the corresponding institution's current rules.

6. FAQ

  • Q: Are care worker and health worker the same role? A: No — the ImmD definition excludes care assistants, health workers and nurses; health workers carry their own statutory registration.
  • Q: Do residential care workers go through ESLS? A: No — the SWD scheme manages quotas separately from the Labour Department's general ESLS route.
  • Q: Can hospital support staff use the residential scheme? A: The scheme covers residential care workers; hospital roles are verified by formal title and duties.
  • Q: Does finishing a course equal registration? A: No — registration follows; health workers apply to the SWD after completing an approved course.
  • Q: Does care experience qualify for any care role? A: No — qualifications are checked role by role against registration and practising requirements.
